Accreditation Survey
Institutional re-accreditation requires that each unit
(academic and support) participate in the compliance portion of the Self-Study
in preparation for the Spring 2009 visit by the HLC evaluation team. Unit
draft documents are required by Spring 2007.
Form Directions: Please follow these directions when using the
templates.
NOTE: These forms will not work properly when using a Mac. If
you need a PC to work on when filling out these forms, please call 341-6923 to
schedule the use of the Accreditation Work Room. Also feel free to use
one of the many Computer Learning Center's found on campus.
- Open Microsoft Word to a blank (new) file - do not open the Self Study form
itself
- Go to the Tools menu
- Click on Macros
- Click on Security
- On the pop-up box, click on Medium
- Close Microsoft Word (completely close down - this sets the security)
- Open the Self Study form
- When prompted with the pop-up box, choose "Enable Macros"
- The form will open and allow for content entry
- To copy and paste content into the text boxes please use Ctrl V (press and
hold the Ctrl key and hit the V key). Please do not use edit and past from the
Microsoft toolbars as this will paste/edit over the document and not inside the
textbox.
- Graphs and tables will have to be indicated within your content as a
reference to a supporting document/material that you will attach in an email to
accredit@mst.edu along with the criterion
form.
- When the form is complete save it using the following naming convention:
<deptname>_Criterion<#>Form.doc (Enter your department name to the beginning
of the form/template name followed by an underscore.)
- Email the following items to the accredit@mst.edu mailbox: a. The complete,
saved (renamed using the naming convention in Step12) form, and b. Any
additional supporting materials for the form being submitted
- Email any questions or concerns to accredit@mst.edu
